Completed SF1449 is required with bid and bid must be within spec of the Statement of Work to be considered. USMCFP SPRINGFIELD MEDICAL WASTE DISPOSAL: STATEMENT OF WORK Subject The Federal Bureau of Prisons intends to make a single award to a responsible entity for the provision of Medical Waste Disposal for USMCFP Springfield. Medical Waste Disposal services shall only be used for medical hazardous waste that cannot be disposed of with garbage removed by the standard trash service. This contract is for Medical Waste Disposal that will need to be provided every Monday, Wednesday, and Friday throughout the contract. Place of Performance Services for Medical Waste Disposal will be performed at the address 1900 W Sunshine St. Springfield, Missouri 65807. There is a commercial entrance off Sunshine Steet to access the property. The driver will proceed to the talk box and receive directions from the officer. There is a small structure that houses medical waste. Period of Performance USMCFP Springfield is soliciting for a 12-month period of performance to start date of award and end 12 months after date of award with a 6-month option to extend to 18 months after date of original award. Schedule The contractor shall pick up Mondays, Wednesdays, and Fridays for the duration of the contract. Any deviation from this schedule will be mutually agreed upon by the Contractor and the Government. There will be no pickups on federal holidays. Pickups scheduled for federal holidays will be rescheduled for the next business day. Note: Three pickups will be required each week per policy. The time of pick up will be from 7:30 am to 10:30 am or 11:30 am to 2:30 pm. Licensing Requirements The institution requires all applicable Federal, EPA, and Missouri licensing / regulations that are standard for this industry. Service must also comply with the Disposal of Patient Materials standards which I have attached below: Security Clearance Services will be provided outside the secured confines of the institution. Please see the Terms and Conditions for any further requirements. Description of Duties The objective of this contract is to provide Medical Waste Disposal for USMCFP Springfield. The contractor shall be responsible for, but is not limited to, the following: The contractor shall be responsible for the collection, removal, and disposal of infectious medical waste. These duties must be in accordance with all Federal, State, City ordinances, laws, and regulations. Containers to furnish will be red, reusable plastic tubs with lid with overall dimensions of not to exceed (24x24x24 inches). Preferably 31 Gallon in size tubs. Tub liners and tape will be furnished by the contractor and appropriately supplied with each pickup. The contractor will leave an appropriate number of tubs and lids to exchange upon request.
